As you can see from the above chart, the Total Net Assets (Financial Strength) of these 20 Florida Hospital Organizations increased by an exceptional 90% to a massive $30.282 bil so far during the Obama Administration.
Also as you can see from the above chart, the Total Net Assets (Financial Strength) of the 13 Non-Profit Hospital Organizations which are not Public Governmental ones increased by an exceptional 91% to $21.306 bil so far during the Obama Administration. Further, in just the past 6 months, these 13 Florida Hospital Organizations had this Total Net Asset growth increase substantially from 63% to 91%.
In addition, the Affordable Care Act (ACA) has also played a key role in this substantial increase in Net Assets (Financial Strength) of these 20 Florida Hospital Organizations.
A remarkable thing is that these very robust increases in Net Assets (Financial Strength) of these Florida Hospital Organizations occurred when these Hospitals were also playing very instrumental roles in the substantial annual percentage growth reduction of Total US Health Care Costs which occurred in each of the most recent three years under the ACA.
With such substantial Net Asset percentage increases of these 20 Florida Hospital Organizations, a key beneficiary of this has been and will continue to be Florida citizens electing to buy health insurance on the Health Insurance Exchange. This exceptional Net Asset growth gives these 20 Florida Hospital Organizations the financial flexibility to moderate their pricing for hospital procedures in their negotiations with health insurance companies which ultimately determines what insurance premium prices are set at by health insurance companies on the Health Insurance Exchange.
Florida Safety Net Hospitals: Public Health Trust of Miami-Dade County
There are a number of Florida Safety Net Public Hospitals. The largest is the Public Health Trust of Miami-Dade County, which is overwhelmingly the huge Jackson Hospitals.
From reviewing its financial statements on EMMA, the Public Health Trust of Miami-Dade County generated an Operating Loss of a large $339 mil in the fiscal year ended September 2012. For the most recent 5 years combined, these Operating Losses totaled a monstrous $2.247 bil.
So Many Struggling Florida Hospitals
But it's not just the Miami area Jackson Hospitals which are severely struggling in the State of Florida Hospital universe.
The State of Florida is being very transparent with its citizens by having available for review at its State Dept of Health website (MyFlorida.com) very detailed financial information on each of its 173 Hospitals. This Operating Statement information presented was for 2011.
Overall, these 173 Florida Hospitals had a Total Operating Income of $1.202 bil in 2011 which was a seemingly respectable Goldilocks 3.9% of their Total Operating Revenues of $31.156 bil. And these 173 Florida Hospitals' Total Operating Income of $1.202 bil in 2011 was burdened with an Earnings Charge of a massive $4.319 bil due to Total Provisions for Bad Debts. In addition, their Total Uncompensated Charity Care Charges Foregone were $6.217 bil in 2011.
But you have to stratify these Hospitals to find out what is really going on with Florida Hospitals.
First, let's look at the upper end Florida Hospitals.
There were 56 of these 173 Florida Hospitals which generated Operating Income as a Percentage of Total Operating Revenues of above 7.5% in 2011. I'll call these Top 56 the Orange Supreme Hospitals. In the aggregate, they generated Total Operating Income of an obscene 12.7% of Total Operating Revenues.....that's more than 25% higher than what the 30 Dow Industrials combined generated on an After-tax basis. This is flat out crazy and tells you why US Health Care Costs have been so incredibly out of control. Here are the Top 56:

But you get a completely different story when you look at the bottom portion of these 173 Florida Hospitals.
In total, 67 of the 173 Florida Hospitals had Operating Losses in the most recent year reported (2011). These Losses totaled $903 mil in 2011, which were a huge (14.0)% of the Total Operating Revenues of $6.439 bil related to these 67 Operating Loss Florida Hospitals, as you can see in the below chart:

When you have 90 or 52% of Florida Hospitals with these huge Operating Losses or very modest Operating Income, the responsible thing to do is to deal with them and not ignore them. So far, the State of Florida has irresponsibly decided to ignore them.
The best and only way to substantially strengthen the very poor Operating Results of these so many struggling Florida Hospitals is for the State of Florida to fully expand Medicaid. This action will dramatically improve the profits of every single Florida Hospital. If Florida fully expands Medicaid, there will be a resultant massive inflow of Patient Revenues, after all there were a massive $4.319 bil of Total Provisions for Bad Debts Charges that reduced the Total Operating Revenues of all Florida Hospitals in 2011. In addition, there were $6.217 bil of Uncompensated Charity Care Charges Foregone in 2011.
A key side benefit here to Florida citizens is that the resultant profit enhancements and thus Net Asset enhancements of all of these Florida Hospitals due to Medicaid expansion will give these Florida Hospitals the financial flexibility to moderate their pricing for hospital and other health care procedures in their negotiations with health insurance companies which ultimately determines what insurance premium prices are set at by health insurance companies on the Health Insurance Exchange.
Also from EMMA, from a review of audited annual financial statements shown at Electronic Municipal Market Access (EMMA), I found 10 Non-Profit, Non-Public Florida Hospital Organizations and 5 Non-Profit Governmental Florida Hospital Organizations with Net Assets above $400 mil each and which have shown full year audited financial statements for each of the most recent 4 years.
The 10 Non-Profit Non-Public Hospital Organizations generated Total Bottom Line Net Income in 2013 which was an amazingly high 11.1% of Total Operating Revenues, which was up a bit from the comparable also amazingly high 9.6% Income Margin in 2012. For the most recent 4 years combined, this Total Bottom Line Profit Margin was a still much too high 8.9%.
The main drivers of this sky high 11.1% Total Profit Margin in 2013 were Clearwater-based BayCare Health System at 24.6%, Coral Gables-based Baptist Health South Florida at 17.1%, Jacksonville-based Baptist Health System at 13.3%, Lakeland-based Lakeland Regional Health at 11.3% and Gainesville-based Shands Teaching Hospitals and Clinics at 10.9%.
All five of them had Bottom Line Profit Margins exceeding that of the 30 Dow Industrials combined. If I were treated at any Hospital that is part of one of these 5 Florida Hospital Organizations, I wouldn't be happy with their outrageous pricing, which bolstered their Bottom Line Profits and was also added to both their Net Assets and treasure chest of Investments in Stocks and Bonds.

There is a very positive aspect to these excessively high Bottom Line Profits of these Florida Non-Profit Hospital Organizations. These incredibly high Profits are getting added to the Net Assets (Financial Strength) of these Florida Hospital Organizations.
Thus a key beneficiary of this should be Florida citizens electing to buy health insurance on the Health Insurance Exchange. This exceptional Net Asset growth driven by these exceedingly high Bottom Line Profits gives these Florida Hospital Organizations the financial flexibility to moderate their pricing for hospital and other health care procedures in their negotiations with health insurance companies which ultimately determines what insurance premium prices are set at by health insurance companies on the Health Insurance Exchange.
And armed with audited financial data concerning the excessively high profits earned by these large Florida Non-Profit Hospital Organizations in the past 4 years, Health Insurance Companies should be able to do a much better job in representing their policy holders, including those getting Health Insurance on the Health Insurance Exchange.
The State of Florida is being very transparent with its citizens by having available for review at its State Dept of Health website (MyFlorida.com) very detailed financial information on each of its 173 Hospitals. This Operating Statement information presented was for 2011.
Overall, these 173 Florida Hospitals had a Total Bottom Line Net Income of $2.047 bil in 2011 which was 6.6% of their Total Operating Revenues of $31.156 bil. This is much higher than the Total Operating Income of $1.202 bil in 2011, which was 3.7% of Total Operating Revenues.
These 173 Florida Hospitals' Total Bottom Line Net Income of $2.047 bil in 2011 was burdened with an Earnings Charge of a massive $4.319 bil due to Total Provisions for Bad Debts. In addition, their Total Uncompensated Charity Care Charges Foregone were another massive $6.217 bil in 2011.

As you can see in the above chart, 13 of the 173 Florida Hospitals had Bottom Line Net Income Profit Margins above 22%, 27 of them above 15% and 52 of them above 10%. And 2011 was a substantially lower profits year for Florida Hospitals than either 2013 or 2012.
If Florida were to elect full Medicaid expansion, all Florida Hospitals would substantially boost their profits. This is where the Provision for Bad Debts and Uncompensated Charity Care Charges Foregone are critical for proper analysis.
Let me just layout the Bottom Line Net Income, Total Operating Revenues and the related Bad Debt Provisions and Uncompensated Charity Care Charges Foregone for a handful of larger Florida Hospitals in 2011:

Reviewing the above chart, the first Hospital.....Safety Net Jackson Memorial Hospital.....had a massive Operating Loss which was funded by Miami-Dade County citizens and by tourists, resulting in a Bottom Line Net Loss of a substantially lower $33.891 mil. Being a Safety Net Hospital, look how incredibly high its Bad Debts Provision of $634.611 mil is. If Florida fully expands Medicaid, a very significant portion of that huge Bad Debts Provision of $634.611 mil would get eliminated, thereby substantially increasing both Operating Revenues and Bottom Line Net Income. Likewise, with full Medicaid expansion, a portion of the $448.046 mil of Uncompensated Charity Care Charges Foregone would result in bolstering both Operating Revenues and Bottom Line Net Income.
Now let's analyze the second Florida Hospital shown above the Broward Health Medical Center, which has a Bottom Line Net Income of $45.534 mil in 2011, resulting in a Bottom Line Net Income Margin of a high 10.9%. It also has $176.721 mil of Bad Debts Provision which has brought down its Bottom Line Net Income to $45.534 mil. Further it has Uncompensated Charity Care Charges Foregone of another $201.337 mil. If Florida fully expands Medicaid and nothing else were to change, this Florida Hospital's Bottom Line Profit % surges from 10.9% to certainly way north of 30%.
And this is the real beauty of full Medicaid expansion. It increases every Hospital's profits so much that Hospitals just have to reduce their Hospital charges very substantially and they also will be much more reasonable in negotiating Hospital Charges with Health Insurance Companies. Thus it is only logical that this will also result in much lower Health Insurance Premiums on the Health Insurance Exchange.
