As you can see from the above chart, the Total Net Assets (Financial Strength) of these 31 California Non-Profit Hospital Organizations increased by a phenomenal 92% to a massive $78.115 bil for their average 4.50 years so far during the Obama Administration.
The 5 University of California Medical Centers with a Total Net Asset growth rate of 45% are a drag on this 92% overall growth rate. In fact, the 26 California Hospitals with these 5 UCal Medical Centers excluded generated a Net Asset growth rate of a higher 98%.
It should be pointed out that the huge, very well run Kaiser Permanente just released its first quarter 2014 Bottom Line Earnings, which were an exceptional $1.1 bil, up substantially from the $765 mil it earned in the first quarter 2013. Yeah, the Affordable Care Act, the Obama Administration and Janet Yellen have all been very kind to Kaiser Permanente, especially in the first quarter of 2014. This $1.1 bil of earnings were not included in Kaiser Permanente's Net Assets above since it hasn't yet disclosed its Total Net Assets at March 31, 2014.
To show how explosive the very recent Net Asset growth was, just in the last six months, the Total Net Asset growth so far during the Obama Administration increased from 61% six months ago to 92% now. Now that is what I call very recent staggering acceleration of Net Asset growth. Kaiser Permanente was the main driver of this massive Net Asset growth upward acceleration in the most recent six months, due to a huge pension liability reduction. In the earlier parts of the Obama Administration, Kaiser Permanente's huge pension adjustments went in the other direction.
Very positively impacting this very robust increase in the Net Assets (Financial Strength) of these 31 California Non-Profit Hospital Organizations were actions taken by both the Obama Administration and the US Fed to strengthen the US Financial Foundation which was severely damaged from the financial meltdown in late 2008.
In addition, the Affordable Care Act (ACA) has also played a key role in this very impressive increase in Net Assets (Financial Strength) of these 31 California Non-Profit Hospital Organizations.
A remarkable thing is that these very robust increases in Net Assets (Financial Strength) of these Non-Profit Hospital Organizations occurred when these Non-Profit Hospitals, and especially the California ones, were also playing very instrumental roles in the substantial annual percentage growth reduction of Total US Health Care Costs which occurred in each of the most recent three years under the ACA.
With such impressive Net Asset percentage increases of these 31 California Hospitals, a key beneficiary of this will be California citizens electing to buy health insurance on the Health Insurance Exchange. This strong Net Asset growth gives these California Hospitals the financial flexibility to moderate their pricing for hospital procedures in their negotiations with health insurance companies which ultimately determines what insurance premium prices are set at by health insurance companies on the Health Insurance Exchange.
And full expansion of Medicaid in California likewise yields a substantial increase in profits and thus also in Net Assets in all California Hospitals, which likewise gives these California Hospitals the financial flexibility to moderate their pricing for hospital procedures in their negotiations with includinghealth insurance companies which ultimately determines what insurance premium prices are set at by health insurance companies on the Health Insurance Exchange.
Large California Hospital Organizations Not Included in the Above 31
From a review of the State of California Health Dept website, there were 4 Publicly-traded Hospital Corporations which had more than $300 mil of Total Net Assets in their California Hospitals at December 31, 2011;
- Tenet HealthCare had more than $1,031 mil in Net Assets at December 31, 2011 including Desert Regional Medical Center in Palm Springs $185 mil, Doctors Medical Center of Modesto $156 mil, Fountain Valley Regional Hospital $133 mil, Twin Cities Hospital in Templeton $129 mil, Sierra Vista Regional Medical Center in San Luis Obispo $125 mil and San Ramon Regional Medical Center $112 mil.
- HCA had $849 mil in Net Assets at December 31, 2011 including Riverside Community Hospital in Riverside $559 mil and Good Samaritan Hospital of San Jose $231 mil.
- Kindred Hospital had more than $499 mil in Net Assets at December 31, 2011 including La Mirada $151 mil and Westminister $115 mil.
- Universal Health Services had more than $307 mil in Net Assets at December 31, 2011 including Southwest Health Care System $165 mil.
Also from its audited financial statements, Michigan headquartered Non-Profit Hospital Organization giant Trinity Health (now CHE Trinity Health) had $510 mil of its Total Net Assets in Fresno-based Saint Agnes Medical Center at June 30, 2013.
From the California State Health Dept website, Colorado headquartered Non-Profit Hospital Organization Sisters of Charity Leavenworth had $428 mil of its Total Net Assets in Santa Monica-based St. John's Hospital and Health Care Center at December 31, 2011.
From the California State Health Dept website, 2 California Safety Net Hospital Organizations had Net Assets above $400 mil in 2011.....Santa Clara Valley Health and Hospital System, part of the County of San Jose, $548 mil and Laguna Honda Hospital, part of the City and County of San Francisco, $476 mil.
There were 7 California Non-Profit Hospital Organizations with Total Net Assets just short of $400 mil, as follows:
- Eisenhower Medical Center in Rancho Mirage.....$399 mil at September 2013
- Washington Township Hospital Healthcare District in Fremont.....$399 mil at Sept 2013
- Palomar Health in Escondido.....$380 mil at June 2013
- Kaweah Delta Health District in Visalia.....$361 mil at June 2013
- Children's Hospital and Research Center in Oakland.....$351 mil at Sept 2013
- San Antonio Community Hospital in Upland.....$349 at Sept 2013
- Salinas Valley Memorial Health Care System in Salinas.....$339 mil at Dec 2011
Los Angeles County Hospitals
Los Angeles County Hospitals are Non-Profit Governmental Entities.
Since they are safety net hospitals and LA is so huge, they have all accumulated huge losses. Below here from EMMA are these losses for each of these 4 LA County Hospitals for the two most recent financial statements.....Fiscal Year Ended June 2013 and 2012:

So for just the one year 2013, these 4 LA County Hospitals posted Total Bottom Line Losses of a massive $0.983 bil and for the one year 2012, the Total Bottom Line Losses were another $1.074 bil.
And it's not just the LA County Hospitals with Huge Losses. Also from EMMA, San Francisco City and County General Hospital generated Total Operating Losses of $152 mil in the 2 most recent years and their Laguna Honda Hospital posted Total Operating Losses of $116 mil in the most recent 2 years.
But the very positive aspect to this is that the State of California has fully expanded Medicaid. So with the resultant massive inflow of Patient Revenues from Medicaid expansion, these 4 LA County Hospitals and the 2 San Francisco City and County Hospitals should be able to turn these massive losses into profits.
One of the key beneficiaries of this very healthy improvement in the Total Net Assets of all California Hospitals due to Medicaid expansion in California will be California citizens electing to buy health insurance on the Health Insurance Exchange. This exceptional Net Asset growth gives all California Hospitals the financial flexibility to moderate their pricing for hospital and other health care procedures in their negotiations with health insurance companies which ultimately determines what insurance premium prices are set at by health insurance companies on the Health Insurance Exchange.
So Many Struggling California Hospitals
The State of California is being very transparent with its citizens by having available for review annually at its Health Information website very detailed financial information on each of its Hospitals. The most recent financial information is for 2011.
Let me first address the Huge Kaiser Foundation Hospitals in California since it didn't break out the Operating Results by Individual Hospital. Instead it disclosed Total Operating Results for its two California Regions, as you can see here below:

As you can see in the above chart, Kaiser Foundation Hospitals generated Total Operating Income of $760.7 mil in all of California in 2011 on $13.282 bil of Total Operating Revenues for a Margin of 5.7%. And all of their Operating Profits were made in their Northern California Hospitals. But a Total Operating Income Margin of 10.0% of Total Operating Revenues seems like awfully steep Northern California Hospital and Premium pricing to me.
Let me now focus on the other 414 California Hospitals included in the excellent California Health website.
Overall, these 414 California Hospitals excluding the Kaiser Hospitals had a Total Operating Income of $3.468 bil in 2011 which was a seemingly respectable Goldilocks 4.3% of their Total Operating Revenues of $79.800 bil. And these 414 California Hospitals' Total Operating Income of $3.468 bil in 2011 was burdened with an Earnings Charge of $4.831 bil due to Total Provisions for Bad Debts. In addition, their Total Uncompensated Charity Care Costs Incurred were also just huge in amount.
But you have to stratify these Hospitals to find out what is really going on with California Hospitals.
First, let's look at the upper end California Hospitals.
There were 93 of these 414 California Hospitals which generated Operating Income as a Percentage of Total Operating Revenues of above 10.0% in 2011. I'll call these Top 93 the Pure Gold Hospitals. In the aggregate, they generated Total Operating Income of 15.6% of Total Operating Revenues.....that's more than 50% higher than what the 30 Dow Industrials combined generated. This is flat out crazy and tells you why US Health Care Costs have been so incredibly out of control. Here are the Top 93:

Anyway, when you have 37% of your State Hospitals generating Operating Losses, you have a financial problem with your State's Hospitals that you have to quickly address.
By far the best and only economically good way to put a major dent in the Huge Operating Losses of the so many California struggling Hospitals is for the State of California to fully expand Medicaid, which they wisely have done. Since California did this, there will be a resultant massive inflow of Patient Revenues from Medicaid expansion, after all there were $4.831 bil of Total Provisions for Bad Debts Charges that reduced the Total Operating Revenues of all California Hospitals in 2011. In addition, there were huge Uncompensated Charity Care Costs Incurred in 2011.
With this massive inflow of Revenues due to Medicaid expansion in California, the struggling California Hospitals will be able to substantially reduce their huge Operating Losses and a huge majority of them should even be able to turn their substantial losses into profits. And due to Medicaid expansion, the California Hospitals with very modest Operating Income will have their profits bolstered very robustly, as will the California Hospitals now making higher than modest Operating Income.
Also since California fully expanded Medicaid, another key beneficiary of the resultant very healthy profit improvement and thus also Total Net Assets improvement of all California Hospitals will be the California citizens electing to buy health insurance on the Health Insurance Exchange. This exceptional Net Asset growth from Medicaid expansion should in all fairness give all California Hospitals the financial flexibility to moderate their pricing for hospital and other health care procedures in their negotiations with health insurance companies which ultimately determines what insurance premium prices are set at by health insurance companies on the Health Insurance Exchange.
