How have these 117 Missouri Hospitals done on the Bottom Line Earnings front? Well the bottom rung of these 117.....and there are so many of them.....have done just horribly.
Of the 117 Missouri Hospitals, 34 of them, or 29% of them, posted Bottom Line Losses Before Extraordinary Items in 2012. And a higher 38 of them registered Operating Losses Before Extraordinary Items.
But more to the point, there were 50 of these 117 Missouri Hospitals, or 43% of them, with Net Assets below $20 mil. These 50 financially weak Missouri Hospitals posted Total Bottom Line Profits Before Extraordinary Items of $12.139 mil in 2012, which was an exceedingly weak 0.8% of their Total Operating Revenues of $1.489 bil in 2012.

And it's the same story with the smallest Missouri Hospitals viewed in terms of the number of beds. While individual Missouri Hospitals are doing very well, in the aggregate, an average 5.5% Profit Margin for the 17 largest Hospitals and 4.5% for the next largest 39 Hospitals is below that of most other US States. But the real story here is that the 37 smallest Missouri Hospitals with bed size of less than 50 posted an average Bottom Line Profit of a very modest 2.1% of Total Operating Revenues. When you combine all of the above facts, the clear conclusion is that there are so many struggling Hospitals in Missouri, and they are all shouting out for a huge dose of financial help.
By far the best solution here is for the State of Missouri to Expand Medicaid.
We love watching the St. Louis Cardinals and the Kansas City Chiefs, but frankly I think that by Missouri Government Leaders refusing to Expand Medicaid, they are not acting in the best interests of a clear majority of their State's citizens. Let me explain.
First, it's the present high percentage of Missouri residents who are either Uninsured or Underinsured, most of whom would be getting fair and decent health care coverage with the Affordable Care Act in combination with the Full Expansion of Medicaid.
All Missouri Hospitals, but especially ones providing emergency care to the many indigent populations in the Urban areas as well as in the smaller rural areas, will have their Bottom Line Earnings as well as their Net Assets, or measure of financial strength, substantially bolstered with Full Medicaid Expansion.
There are specifically two Hospital Earnings Charges which will be substantially reduced with the Affordable Care Act, and especially in combination with Full Expansion of Medicaid in Missouri......the Provision for Bad Debts and Uncompensated Charity Care Charges.
OK, that should make sense but just how large are those two Earnings Charges now?
Well, the cool thing here is that with its financial transparency, the State of Missouri's Dept of Health has disclosed the amounts of these two Earnings Charges for each of its 117 Missouri Short-term Acute Hospitals.
All 117 Missouri Hospitals generated Total Bottom Line Earnings Before Extraordinary Items of $851 mil in 2012, which included an Earnings Charge of $1.238 bil due to Provisions for Bad Debts. Thus their Total Earnings Before these Bad Debt Charges were $2.089 bil. In addition, their Total Uncompensated Charity Care Charges Foregone were another $1.538 bil in 2012.
Granted all of these two huge Earnings Charges will not be eliminated with Full Expansion of Medicaid in Missouri, but a substantial portion of them will be.
And second, there are many of these Missouri Hospitals, such as the huge Truman Medical Center in KC ($207 mil of Uncompensated Charity Care Charges Foregone in 2012), which are Government Hospitals, with many of them being at least partially financed with taxes paid by Missouri citizens. Full Expansion of Medicaid should substantially improve the Earnings and Losses now being registered by these Missouri Governmental Hospitals. Thus it is only logical that not as many taxes will be needed after Full Expansion of Medicaid in Missouri. So it's not just the presently Uninsured and Underinsured who will benefit from the Full Medicaid Expansion in Missouri. So will all Missouri taxpayers who in all fairness should be getting tax cuts.
