25% of North Carolina Hospitals Post Losses Pointing to Need for Medicaid Expansion

In an earlier recent post North Carolina Smaller Non-Profit Hospitals Weak Profits , from a review of audited financial statements in the Electronic Municipal Market Access (EMMA), I found 27 Non-Profit Hospital Organizations headquartered in the State of North Carolina with Total Operating Revenues of more than $10 mil each in the most recent audited fiscal year. 

In this earlier study, the 13 smaller North Carolina Non-Profit Hospital Organizations with Net Assets below $400 mil generated Audited Total Operating Income as a % of Total Operating Revenues of a weak 1.9% in the most recent fiscal year.  This compares to a higher 4.6% for the 14 large ones with Net Assets above $400 mil.

In this post, I reviewed  certain key data related to the 83 individual North Carolina hospitals contained in the excellent American Hospital Directory's (AHD) website.   I excluded several of the smaller Specialty Hospitals.

What I found was that 21 of the 83 North Carolina Hospitals, or 25% of them, posted Bottom Line Net Losses in 2012, which totaled $76.5 mil.

When you review the above 2012 Average Net Income (Loss) amounts, it is pretty clear that there is massive income inequality between the large and small North Carolina Hospitals......yeah, that's an Average Net Income of $129,198,000 for the 6 largest NC Hospitals, and $57,579,000 for the next 10 largest NC Hospitals as compared to an Average Net Loss of $(7,157,000) for the 13 smallest NC Hospitals and an Average Net Income of only $6,818,000 for the next 37 smallest NC Hospitals.  US Health Care and its giant costs will not be fixed until the profits and losses of both the bottom rungs and the top rungs of the profit and loss ladders of Hospitals all throughout the US are properly and equitably fixed. 

The main driver of these very weak profit numbers for the smaller North Carolina Hospitals were the many uninsured and underinsured residents, particularly in the smaller North Carolina communities, with the resultant abnormally high levels of both Total Bad Debt Earnings Charges and Uncompensated Charity Care Earnings Charges relative to Net Patient Revenues.

In the North Carolina Hospital Association website there is a section on Community Benefits provided by every North Carolina Hospital.  North Carolina Hospitals provided a total of nearly $1 bil of Charity Care and another $700 mil of Provisions for Bad Debts in the most recent year.

The best way to provide the needed financial boost to these small North Carolina Hospitals is to simply Expand Medicaid fully in North Carolina.  This will substantially reduce two Hospital Operating Statement Earnings Charges.....Provisions for Bad Debts and Uncompensated Charity Care Costs.
